Description
Fall prevention is inadequately addressed in primary care leading to disparities in outcomes. Many primary care providers (PCPs) are unaware of their patients' falls or gait disturbance, as half of older people do not discuss. Advances in computing technology can help to identify older people at risk of falls and disseminate guidance about the most effective interventions using clinical decision support (CDS). Development of the eSTEPS CDS within the wide'ly adopted Epic EHR will support dissemination of evidence-based fall prevention and exercise recommendations for older adults. We will focus on a diverse cohort of elders to assist in reducing disparities in outcomes.
