Description
Objectives and Outcomes: This project aims to empower older adults living with chronic conditions and their caregivers to live active, healthy lives by promoting the adoption of healthy behaviors through evidence-based programs. Our main objectives are 1) building the infrastructure of the Chronic Disease Self-Management Education (CDSME) and Self-Management Support programs at the University of Texas Medical Branch (UTMB) and our partners, and 2) Creating conditions for project sustainability, scalability, and replicability. Anticipated outcomes are on a 1) Institutional level (change in institutional policies and behavior/culture, advancing supportive systems, and training of personnel to implement both sets of programs as an integral part of comprehensive management for chronic disease) and 2) Participant level
(change in knowledge, behavior, attitude and clinical outcomes, as well as people-centered).
