note = "Funding Information: Given its success in the VA system, a multicenter study submitted as a patient safety grant through the ACS was funded by the Agency for Healthcare Research and Quality to demonstrate the feasibility of applying the NSQIP methodology to private sector hospitals. 16 In brief, the NSQIP consists of the collection of preoperative risk and laboratory data, intraoperative data, and 30-day postoperative morbidity and mortality data on most patients undergoing major operations under general, spinal, or epidural anesthesia. A surgical nurse reviewer is assigned to collect and transmit data at each center after completing in-depth training on data collection procedures and standardized definitions of each variable. Nurse reviewers then obtain information on outcomes by conducting periodic reviews of the medical record, attending morbidity and mortality conferences, contacting surgical attendings and primary care providers, and communicating with patients after discharge from the hospital by follow-up letter or telephone call. Regular conference calls and periodic site visits are conducted to ensure interrater reliability, data quality, and reliability at each center. ",
