Abstract
Recent stroke clinical trials with positive results have shown the efficacy of endovascular therapy in the treatment of patients with acute ischemic stroke. One major difference between these positive trials and the previous negative ones is the use of imaging biomarkers to select patients for randomization. In upcoming trials, imaging biomarkers will likely be used to identify patient cohorts and optimize their selection for the tested treatment. The purpose of this chapter is to review systematically and critically the imaging biomarkers typically used in stroke clinical trials.
