Abstract
Paraesophageal hernias may present with acute gastric volvulus and obstruction, thus are important for every surgeon taking ED call to be comfortable managing. This chapter reviews the management of acute presentations including initial management, definitive repair and gastropexy, and surgical management of gastric ischemia when it occurs. It is important to know how to turn an emergency into an urgent or even elective surgical problem and how to safely resolve a volvulus with few complications, short and long term. This is a complex problem, but does not require a foregut specialist if one is not available.
