Superior Capsule Reconstruction for the Treatment of Irreparable Rotator Cuff Tears

Abstract

Massive irreparable posterosuperior rotator cuff tears remain a challenge especially in younger patients who have a desire to remain active. Joint-preserving treatment options include partial rotator cuff repair, rotator cuff repair with graft augmentation, implantation of a subacromial spacer, and tendon transfers. For those with rotator cuff arthropathy or concomitant irreparable subscapularis tears, reverse total shoulder arthroplasty may be a better option. Superior capsule reconstruction (SCR) has shown promising biomechanical and clinical results in the treatment of these tears. Despite the limited number of published outcomes, superior capsule reconstruction is showing encouraging results for this difficult problem. In this article, we describe our technique for arthroscopic superior capsular reconstruction.
